We accept all insurance plans. Prior Authorization is required before your appointment. If you would like to use insurance, this must be prearranged. We will need 1 to 2 days to do this before you can be scheduled. The information can be faxed to us at 602-230-9025. Please call the office to make these arrangements. We will work with you to maximize your insurance benefits.
We accept cash, MasterCard (credit or debit), Visa (credit or debit), cashier's check from a bank, travel's check and money order with ID. The person whose name is on the card or check must be present to sign. We do not accept personal checks. We can arrange payment from a third party the day before the appointment via fax. Please be prepared to pay in full upon arrival.
